Community Action for Prenatal Care  |  Community Health Worker Program

The Buffalo Home Visiting Program provides comprehensive home based services to families for 3 to 5 years after a child is born. The program has a single goal: to help parents meet the challenges of parenting in order to assure healthy development for their children.

Part of a nationwide initiative called Healthy Families America (HFA), The Buffalo Home Visiting Program is based on over two decades of research and experience with other home visitor programs.

The Buffalo Home Visiting Program:

• Ensures that families' basic needs are met (help is given to families who need services from food pantries, public assistance, WIC, housing assistance, etc.).
• Provides families with a family support worker who will visit them in their home once a week to provide educational information and resources.
• Promotes positive parent child interactions by educating families on child development and by providing activities to help them enhance their child's growth and development.
• Strengthens the family support system by linking them to appropriate programs and agencies.
• Empowers and educates families to enable self-advocacy.
